Canfield Kids requires a completed enrollment packet prior to attending. Enrollment packets can be
submitted through email or placed in the MHEF mailbox outside of the main office or the Civic Center. The
packet includes this handbook, contact information, medical emergency authorization, photo release,
immunization records and medical status, and information to parent’s policy, as required by The
Department of Children and Families. Additional forms may be included at the discretion of
Canfield Kids. Applications for enrollment are accepted without regards to race, religion, sex, or
national origin. Canfield Kids reserves the right to refuse service to any family or child who does
not follow the policies and procedures of the handbook, or poses an emotional or physical threat to
other children.
